Student Health & Safeguarding Forms

A number of forms, including the Student Health Form and safeguarding forms, have been updated in line with the General Data Protection Regulations. These can be found in your 'Resources' members area on the BWY Website. We have provided templates for forms that you may wish to use for teaching purposes. As a self-employed teacher it is your responsibility to ensure that you use and retain these forms in a compliant manner. Blue Fin Insurance have informed us that the only reason you would need to retain a Student Health Form when a student no longer attends your class is if there has been an Accident Report Form completed for that particular student. If you send a copy of the Student Health Form to us, with the Accident Report Form, we can ensure it is retained for the appropriate length of time.

Please note the Accident Report Form has also been updated and should be used for future reporting.
